Probably he kicks himself the most for not tipping Maeglin over the city walls when he had the chance. Or at least tying the elf up and leaving him outside the city gates when he started to look odd.
Ace: I'm gonna go with 'told the ultimate evil the answer to the riddle the Doctor was using to trap him'. Oops?
Haymitch: Not dying. That, or mouthing off to President Snow after he failed to die. He's not sure which one caused more damage.
Balthazar: Being too slow to stop Veronica from doing what she did. There had to have been a better way to contain Morgana while they regrouped.

Charlie: Taking up smoking again after quitting for a while.
Cyborg: Nothing as of yet, but he's young.
Gibbs: Enlisting. He should have just hopped a ship to the tropics and been done with it.
Knox: I don't think he's ever done a Big Stupid. More like a lot of little ones that never quite get that bad.
Howard Stark: At the moment, not making sure he could find a way to run his company while serving with the Strategic Scientific Reserve. Worse mistakes are likely to come.

Mia: Probably just being slow on the uptake when she was 16 in general. If she had just stopped to think about it and acted sooner, her mother wouldn't have spent months being tortured, she would've caught on to Ghaleon's plans before Alex and co. had to tell her, and she would've been able to stop Nash's traitorous deeds. But if all of that happened, we wouldn't really have a game.
Lucas: There's a point when Lucas and co. get to a needle before the Pigmask Army. However, instead of going over there and pulling the damn needle first, they allow the Pigmask Army to do their little fanfare red carpet thing for the Masked Man and he pulls it right in front of them. WHAT.
Lohengrin: In his opinion? Falling in love. That's just going to mess everything up for him. But that's up there with letting it get to the point that Siegfried has to shatter his own heart, though that's more Prince-stupid than Knight-stupid if you ask him.

SiegfriedMytho's heart again may have been Fakir's second-stupidest action.The stupidest was that time that six-year-old Fakir tried to be a hero and got lots of people, including both of his parents, killed. It's hard to beat that one.

Tavi: First two pages of the series, at fifteen years old: volunteering to pick flowers for a girl who didn't even like him.
Lois: Current time: the many minor infractions, ending in a drinking one and being framed for murder (and then nearly murdered herself) that got her kicked out of Metropolis University. By end of canon: nearly gave Zod the means to end the world.

Arguably, Shawn's biggest Stupid Move was lying about being a psychic in the first place, though that hasn't come back to bite him in the ass too badly. (Though it's probably only a matter of time. :P) More recently and specifically, it would either be getting roped into a fake think tank run by a would-be assassin, or accidentally outing someone from federal witness protection. (He... kind of had a run of epically bad decisions in season four. :P)
And Fry... well, it's hard to beat accidentally becoming your own grandfather.

Vriska retaliated by mind-controlling Aradia's boyfriend (quadrant uncertain) into killing her. Sometimes you just can't win.
This might have been part of the voices' plan, but on Aradia's part... it was a noble gesture, but pretty much completely futile. Vriska didn't even care about killing her, although at an earlier point she seemed kind of upset about how dead!Aradia didn't give a damn about her.

Clementine is basically the Goddess of Bad Choices. She's done everything from drive drunk and steal a perp's pot on camera to instructing teenagers in the art of cursing someone through dark magic to accruing a massive amount of student loan debt to selling nude photos of herself online with her face fully visible. She believes far true strongly in fate and karma, which causes her to utter this aloud.
Dixie would say marrying Doc, because her time with him was wasted time in nearly every respect; the best thing he did for her was hook her up with other gang contacts, which she ultimately used to aid those on the side of goodness.
Juliet: Getting kidnapped by Yin. She still blames herself for that one.
Pinkie Pie: Pinkie's repeatedly incited chaos without showing too much regret (what happened on Nightmare Night and during Gummy's unbirthday party, for instance) and even when she feels bad, she shakes it off by laughing at her own foibles. I think she'd say her biggest stupid moves involve hurting others, chiefly being unable to get Cranky to forgive her.
