Avoiding a certain movie... I played Charlie Pace from Lost for years here. When he died in the show, it was pretty hard to take at first, but I decided to have him die permanently in the bar as well since it felt like a good and proper end. I still miss him as a head voice sometimes.
Same with Ianto Jones, who I played for a long time, too.
In-canon deaths tend to kill heads voices for me a lot, it would seem, unless I start out the character as dead in-game.

We'd been working from the beginning under the assumption that Kanan would die in the final season. So his death wasn't a shock, we were pretty well-prepared for it at that point. And setting aside our misgivings about some of the circumstances, none of which were really about Kanan, we're fine with how it happened. But it was, still, upsetting when it happened. In part I think because when we started writing these characters with each other, we were really far apart (as in, opposite-sides-of-the-planet far apart), and even though we had talked and everything, writing these characters together was like an emotional opportunity for us to be together even when we were apart. So with that kind of emotional intensity involved, even fully expecting this outcome and being well-prepared for it, and really believing it was the right thing for the story – it was still pretty hard to take.

And then the story arc on Mandalore, where she had to face up to what she'd created/done and how well she accepted responsibility for it. Damn...I'm getting verklempt..talk amongst yourselves.
Ahsoka: The ending of season 5 of Clone Wars and her talk with Anakin. Hell, that whole arc and how the Jedi counsel failed so miserably. I wanted to scream at Mace Windu and his, "This was your great test" like that made it all OK. (I might still resent that, tbh.)
And the events of "Twlight of the Apprentice". My emotions.
John: The season finale and how all he held dear and built just crumbled.
Past pups...
Ben Grimm: "Fear Itself". I know the story didn't do amazing things, but Ben losing himself and becoming an actual monster was really hard and ended up finalizing my play with him.

Also the ending was frustrating because it left almost everything hanging except the Sburb session and the defeat of the main villains and there's a bunch of characters who aren't even definitely alive or dead.
